Living well programme at Fair Oaks Day Hospice

Walsall Healthcare NHS Trust provides a nurse-led specialist service at Fair Oaks Day Hospice to support people with palliative care needs.

The Living Well Programme provides you with the help and skills to help manage your disease. Their support aims to improve well-being, ensure effective symptom management and reduce unplanned hospital admissions.

The staff at Fair Oaks can also provide support to families and carers of patients with palliative care needs by telephone or a face-to-face meeting.

Any healthcare professional involved in your care is able to refer you to Fair Oaks. Once a healthcare professional refers you, Fair Oaks will contact you for a short telephone triage and arrange a date for your first assessment.
